The John C. McDonald Regional Transplant Center Team strives to provide quality and compassionate care to our patients and their families. Our Team is available to answer questions and provide support throughout the evaluation process, during the transplantation period, and long past the date of transplantation.
All new patients should receive the following paperwork by mail. In the event you have misplaced your paperwork or did not receive it, you can print these forms and complete them prior to your visit.

What is Inform Me?
- Inform Me is a decision aid to help patients make informed treatment decisions about whether to accept or to refuse a kidney from an increased risk donor, that:
- Includes 4 chapters with brief text, videos, and graphics
- Survey questions after each chapter
- Focuses on kidneys, but also applies to other organs
