However, patients should note that hospital based physicians such as: radiologists, pathologists, anesthesiologists, emergency room physicians, hospitalists, and neonatologists providing healthcare services at the hospital may not always have a contract with all the companies listed above. These physicians will submit charges for professional services provided to you and are in addition to the patient’s hospital bill. In order to obtain information regarding network participation of any hospital based physician, patients may refer to the online provider directory of their insurance company for the most up-to-date information, call the hospital admitting department, or call the physician’s customer service number. Patients may request specific information from their health insurance issuer as to what benefits are payable under their healthcare policy for hospital based physicians.
The hospitals will send you a bill for services you received that were provided by the hospital. Physicians who care for you bill their services separately.
In some cases, you may receive bills from doctors who consulted with the physician caring for you, interpreting test results or providing a specialized service. These medical providers may include pathologists, radiologists, anesthesiologists, CRNAs, emergency room physicians, cardiologists, surgeons and other specialists.

YOU MAY BE RESPONSIBLE FOR PAYMENT OF ALL OR PART OF THE FEES FOR THOSE OUT-OF-NETWORK SERVICES, IN ADDITION TO APPLICABLE AMOUNTS DUE FOR CO-PAYMENTS, COINSURANCE, DEDUCTIBLES, AND NON-COVERED SERVICES. SPECIFIC INFORMATION ABOUT IN-NETWORK AND OUT-OF-NETWORK FACILITY BASED PHYSICIANS CAN BE FOUND AT THE WEBSITE ADDRESS OF YOUR HEALTH PLAN OR BY CALLING THE CUTOMER SERVICE TELEPHONE NUMBER OF YOUR HEALTH PLAN.
