Request Copies of Your Medical Records

To request copies of your medical records, please download, complete, and sign the Authorization for Release of Information form for your Willis-Knighton Health Center hospital facility. Completed forms, along with a copy of a valid ID (driver’s license, military ID, or passport), and any other necessary documentation may be mailed to the facility's health information management department.
