Transfer Evaluation Process
The patient transfer evaluation process consists of:
- Introductory Call and Request
- We will ask for pertinent information and conduct an initial screening.
- Evaluation of facility capacity and service availability.
- Done in partnership with each facility administrative and nursing teams.
- Communication with the needed providers for the services requested.
- Collection of information with final transfer acceptance decision communicated back to you.
Patient Information Packet need to include:
- Face Sheet
- ED MD Documentation
- H&P
- Progress Notes
- Labs
- Radiology Results
- Consult Notes
